Lifescape takes applications for two state support programs

February 17, 2010

From press release

Area low-income and senior residents who have applied for the Circuit Breaker and Illinois Cares Rx program in the past must reapply again. Lifescape Community Services is helping those applicants in the area to reapply. You must file an application each year to participate in the Illinois Cares Rx program or else face a possible lapse in coverage.

What is Circuit Breaker and Illinois Cares Rx program? It’s a program that may help pay for prescriptions, a possible cash grant for property tax and rent relief and a discount on your license plate sticker. A newer benefit is the People with Disabilities Ride Free Transit Card.

There are requirements to take part in this program:

→ You must be 65 years of age or older.
→ Under 65 years of age, must be disabled.

Your total income in 2009 must be less than $27,160 if single; $36,635 if filing as a married couple. Assets are not counted for this program.

The Illinois Cares Rx program has also expanded its benefits for those on Medicare. This year, the prescription benefit will cover most medications for individuals on Medicare, regardless of age.

If you need help applying for these benefits or have questions, call Lifescape Community Services at (815) 963-1609. Caseworkers are scheduling appointments at Lifescape’s offices at 705 Kilburn Ave. in Rockford. They will also have various sign-ups in the community and providing visits to homebound individuals.
