SwedishAmerican Health System announces provider relocations

SwedishAmerican Health System has announced the relocation of several providers within SwedishAmerican Medical Group, the organization’s network of primary and specialty care clinics.
March 31, Pediatrician Kendra Hall, M.D., FAAP, relocated her practice from SAMG/Woodside to SAMG/Rock Valley (formerly Rock Valley Women’s Health Center) at 6861 Villagreen View in Rockford.
April 14, OB/Gyn providers Tarek Harb, M.D., FACOG; David Moore, M.D.; David Schleicher, M.D.; and Lois Hull, APN, will relocate from the Brookside Specialty Center to SAMG/Obstetrics and Gynecology, a new clinic on the second floor of the SwedishAmerican Ninth Street Center, 209 Ninth St.
April 25, Nurse Practitioner Kristine Bohrmueller, APN, will relocate from the SwedishAmerican Center For Women’s third-floor location in the Renaissance Pavilion and join Dr. Tarek Harb at SwedishAmerican Medical Group/Obstetrics & Gynecology, 209 Ninth St.
May 30, Nurse Practitioner Lisa Larson, APN, will relocate from the SwedishAmerican Center For Women’s third-floor location in the Renaissance Pavilion to Midwest Heart Specialists, where she will work with cardiac patients.
Current patients are being notified directly about the various relocations, and may contact their provider’s office with any questions.
