SwedishAmerican named Anticoagulation Center of Excellence

SwedishAmerican Health System has been named an “Anticoagulation Center of Excellence” by the Anticoagulation Forum, reflecting a strong commitment to providing the highest level of care to patients taking antithrombotic medications.
“We are very proud to have passed the Anticoagulation Centers of Excellence assessment,” said SwedishAmerican Director of Pharmacy Services E. Thomas Carey, Pharm D. “Our organization is dedicated to actively participating in all aspects of our patients’ care, which is the best way to improve health outcomes.”
The Anticoagulation Centers of Excellence program helps health care professionals provide the highest level of care and achieve the best possible outcomes for patients on antithrombotic medications. The program offers a road map to consistent, sustainable excellence in five key areas of patient care. Because it successfully met the rigorous standards in each patient care pillar, SwedishAmerican will be considered an Anticoagulation Center of Excellence for a period of two years.
“Until now, there has not been a comprehensive standard to define ideal practices for an anticoagulation service or a central place to find relevant resources for our practice,” Carey said. “This educational program will have a lasting and profound effect on anticoagulation care for many years to come, and we are proud to be considered a Center of Excellence.”
SwedishAmerican’s Anticoagulation Clinic is composed of a team of pharmacists certified in anticoagulation care. Patients are referred by physicians within the health system to help optimize anticoagulation therapy patient safety. The clinic performs lab monitoring, education, dosage adjustments and peri-procedural planning. It was established in the spring of 2012, has been growing steadily and continues to accept new patients from SwedishAmerican Medical Group clinics.
